Foreign experience of development of public-private partnerships in the health system


The article examines the experience of introducing public-private partnerships (PPPs) in the field of health care in developed foreign countries. PPP has become an effective tool for addressing health problems in modern conditions. In Ukraine, in the conditions of limited state financial resources, poor quality and availability of health services, there is a growing need for the development of such partnerships. The advantages of the PPP mechanism are considered in the context of the problem under study which contributes, in particular, to reducing the burden on the budget, increasing efficiency in the implementation of projects, and accelerating the solution of socially important tasks in public health. The problems of PPP development are opacity in decision-making processes, inadequate public control and public participation in the process of PPP projects implementation, social justice restrictions, high cost, and complexity of implementation on a large scale. The main forms and models of PPPs are disclosed, the risks of their application are the risk of capital investments, the risk of receipt, market risk, and the risk of transferring risks to the public sector. The factors that restrict the development of PPP, including in Ukraine: organizational, institutional, economic, social and psychological. Recommendations are formulated regarding the prospects for the development of PPPs in Ukraine: developing and adopting legislative initiatives to create transparent, competitive conditions for the functioning of PPPs in health care; the formation of a road map for PPPs; creation of a specialized institution for management and monitoring of PPPs; introduction of model agreements on joint activities within the framework of PPP, concession and lease; formation of methodical recommendations for the selection and use of financial PPP models; registration of objects realized on the basis of PPP in the registers of capital investment objects and state contracts; disclosure in the budget documents and the financial performance report for each PPP project information; implementation of budgetary planning in the evaluation of prospective expenditures for PPP projects and setting of limit levels of these expenditures both within the medium-term planning and in the long-term budget forecast.
